3. Send form to school
     with your child in an
     envelope marked
     accordingly and that
     includes payment.

Forms, flyers and notices about HSA-sponsored activities must be approved by the HSA president.
Send to Wendy or Kara.

Forms, flyers and notices about district-sponsored and other external activities must be approved by Dr. Weber
or Dr. Verducci's office. Send to Lu Jones for approval.

Distribution policy for third-party flyers

Anyone can access the Web. Be careful about what kind of information you post on your form/flyer. See our note about PRIVACY. Allow at least two days for approvals.
